School Compliance Verification - Kentucky
The School Compliance Verification form is necessary for all 16 & 17 year old students who wish to apply for a driver's permit in Kentucky. Additional information is provided below explaining this process and terms of compliance.
Please print the school compliance form and complete the "student information" portion of the form which must be submitted to the registrar. A copy of this form will be kept on file at the school until no longer valid.
Persons have a period of 2 months to obtain this certificate and complete initial driver's testing. Please do not contact the office outside business hours, and plan ahead for student testing outside school days to avoid unexcused absences.
