Applications for the 2019-2020 academic year are now available.
Please call the registrar to obtain an application. Please note that at this time (1/15/2019), all information for Our Lady of the Sacred Heart Academy, and uniform information for both schools, is not yet available.
Only one application must be submitted if your children will attend Assumption Academy, Our Lady of the Sacred Heart Academy, or both.
Page 1-3 of the handbook will guide you through what is necessary for admission, including physicals and state-required forms. You can view our handbook here.
Please complete and mail all forms to the Registrar to arrive by April 15th for returning families (including any new students within that family), and May 1st for new families. Applications with incomplete or missing forms will not be accepted and will incur a late fee if incomplete after the deadline.
New families: you will be contacted for scheduling times for student placement tests in math, language arts, and Latin. Additional work may be sent home to be completed for entry into the appropriate grade level. All applications must be approved by the Principal, so you will be notified if your children are accepted into the academy following the testing.

Tuition contracts are only available from Assumption Academy's registrar. Assumption requires both a paper contract and registration on the FACTS website. Those who would like financial assistance must apply for aid using the FACTS system.
Tuition: Tuition is the price of instruction for students enrolled at the Academy. Various fees, where applicable, are extra as detailed below. Current annual tuition charges are as follows. All tuition and fees may be subject to change.
K – 1st Grade: $2,000
2nd – 8th Grade: $2,500
9th – 12th Grade: $3,500
Discounts: 0% Oldest, 15% 2nd Oldest, 25% 3rd Oldest, 50% 4th Oldest, 5 Oldest and any additional children 100% discount.
Fees:
Late registration fee: (Returning families after 4/15/19; new: 5/1/19) $50
Book Rental Fees: $100 per student
Senior Graduation Fee: (Assessed only in senior year per student) $100
Fundraising requirement: $400 per student.

Immunizations
We do not accept out of state forms. Please read the instructions and download the appropriate forms for the state of Kentucky. Please note that you must submit both the history of shots and the exemption if your child has a shot history and you are now refusing to vaccinate. Please call the registrar with any questions.

Returning Student Admissions
Returning families must ensure their sacramental, medical, and immunization history is up to date. Contract agreement forms are available only from the registrar.

School supply list: Assumption Academy
Students attending Assumption will need blue or black ink pens, loose-leaf paper, notebooks, folders and/or binders, a book bag, gym bag, lunch bag, and a water bottle.
Additional erasers, a ruler, pencil case, white-out, and tissues or handkerchiefs, are all items that students may find useful. The Academy does not provide lunch items, napkins, paper towels outside restroom use, tissues, or other disposable items.
Some teachers may require additional supplies.

Sports physicals
Yearly sports physicals are required for all boys in 5th -12th who wish to play team sports. Physicals apply only to the academic year in which they were obtained, no matter when they were obtained. Although all boys are encouraged to join the Horsemen team in one or all our sports (Soccer, Baseball, Basketball), participation is voluntary. Only the forms below will be accepted. Parents must submit all pages completed to the academy, and the doctor's form must be completed by a licensed physician to be considered valid.
